Hypokalaemic periodic paralysis precipitated by diuretic therapy and minor surgery.

چکیده
Introduction Hypokalaemic periodic paralysis is a rare myopathic disorder characterized by attacks of muscular weakness typically precipitated by exercise or heavy carbohydrate meals. Although usually familial, sporadic cases may occur, often in association with other conditions such as thyrotoxicosis (McArdle, 1974). A patient is now reported in whom the diagnosis of hypokalaemic periodic paralysis was made after minor surgery: prior treatment with a thiazide-like diuretic may have been contributory to the acute attack.

Muscle channelopathies: does the predicted channel gating pore offer new treatment insights for hypokalaemic periodic paralysis?
Hypokalaemic periodic paralysis (hypoPP) is the archetypal skeletal muscle channelopathy caused by dysfunction of one of two sarcolemmal ion channels, either the sodium channel Nav1.4 or the calcium channel Cav1.1. Clinically, hypoPP is characterised by episodes of often severe flaccid muscle paralysis, in which the muscle fibre membrane becomes electrically inexcitable, and which may be precip...

Acute generalized weakness has a wide differential diagnosis that includes neurologic, metabolic, and infectious aetiologies. Acute hypokalaemic paralysis is a rare but treatable cause of acute weakness. We present a 23-year old lady, with history of sudden difficulty getting up from recumbency. Her initial investigations revealed hypokalemia with nonanion gap metabolic acidosis and alkaline ur...
متن کاملNormal insulin release during sustained hyperglycaemia in hypokalaemic periodic paralysis: role of the potassium channel opener pinacidil in impaired muscle strength.
1. Hypokalaemic periodic paralysis is characterized by attacks of muscle weakness. Glucose, insulin and an abnormal regulation of ATP-sensitive potassium channels may be involved in these attacks. We studied the effect of hyperglycaemia and of the potassium channel opener pinacidil on insulin release and muscle strength in patients with hypokalaemic periodic paralysis. 2. Insulin release was as...
متن کاملFamilial hypokalaemic periodic paralysis in Finland.
Seven families with familial hypokalaemic periodic paralysis were found in Finland. Nine of the 103 asymptomatic family members studied had abnormal results on a potassium exercise test. The overall prevalence of familial hypokalaemic periodic paralysis in Finland was 0.4/100,000. Carbohydrate intake and hard exercise were the most important triggers of paralytic attacks. Half of the patients r...
